We want to create services in local communities that are properly joined-up so people only go into hospital when it is absolutely necessary and do not stay longer than they need to.

In each of our communities we are working with local councils to integrate health and social care services and to improve the health of our local people by supporting them to access better housing, better jobs and to live healthier lives.

Groups of organisations are working together in each local area to develop health and care plans for their local populations. These plans will be different for each different area, reflecting the needs and the priorities of local people. They share common themes, focusing on making local health services more joined-up so that people are able to access the right care, in the right place, at the right time.
