Our priorities
We want to improve the health and life expectancy of our population through prevention initiatives and support for people to take care of themselves and their loved ones.
We want to create services in local communities that are properly joined-up so people only go into hospital when it is absolutely necessary and do not stay longer than they need to.
We want to create more efficient hospital-based services for those who need them, making the best use of the resources and workforce across the system to plan and deliver hospital-based services.
We want to ensure that mental health is seen to be equally important as physical health and that the services we offer promote the best mental health for our local population.
We want to help more people to survive cancer and support people in our region to live well with and beyond cancer.

We want to make the most of every penny available to deliver good quality local services within the funding available.
How we achieve these aims
In order to achieve this ambitious programme of change, we have established six place-based programmes of work in each of our six places; the areas covered by the six Clinical Commissioning Groups (CCGs) within our partnership. These place-based programmes are primarily working on achieving the first two priorities: healthier people and better out of hospital care.
In addition, there are three cross-cutting programmes of work where we are working across the whole of the Humber, Coast and Vale area to plan services for our local populations. These programmes will help us to achieve our priorities to improve hospital-based care; mental health care and cancer care.
Finally, we have established programmes of work that will help to make the change happen, we sometimes call these “enablers”. This includes groups working on communications and engagement, workforce and staffing issues, buildings and estates, digital technology and finance. These groups will ensure the basic building blocks needed to deliver change are in place and our programmes of change have the best possible chance of success.
